Double bookings are the responsibility of the delegate. Cancellation of a double booking is subject to an administration fee of 25 %.

* EACTS and ESTS members who have paid their subscription fees for 2006 and have submitted a pre-registration form by 30 June 2006 are entitled to free registration with an administration charge of €10 (non-refundable) to the 5th EACTS/ESTS Joint Meeting. This registration is not transferable.
** To qualify for reduced and concessionary fees, participants must ensure that their Registration Form is signed by their Head of Department, confirming their status. Delegates registering online should fax through a statement from their Head of Department. Delegates registering on-site must bring with them a statement from their Head of Department if they wish to be registered at the reduced or concessionary fee.
The latest date for pre-registration is 18 August 2006.
Registrations received after this date will NOT be processed.
The registration fee for delegates includes:

- Meeting documentation including the Book of Abstracts

The registration fee for accompanying persons includes:
- Welcome Reception on Sunday 10 September
- City Tour on either Monday 11 September or Tuesday 12 September
- Admission to the Presidential Address, Honoured Guest and Basic Science Lectures

PAYMENT
Payment by delegates and accompanying persons for registration, scientific sessions and Presidential Dinner should be made either online or on the pre-registration form and sent to the EACTS Executive Secretariat. All pre-payments must be in EURO (€).
Booking and payment for the Accommodation and Optional Social Activities shoud be made through StoCon.
Registration forms will not be processed without payment. Payments may be made as follows:
- Credit Card - VISA, Mastercard and American Express are accepted.
- Bank Transfer - Transfers made must be exempt of charges to EACTS Trading Company Ltd. In order to ensure that your payment is identified, you must mention the name of the delegate and "5th EACTS/ESTS Joint Meeting".

Payment on-site
The currency accepted for payments on-site will be EURO (€). Payments can be made by cash or credit card.
ACKNOWLEDGEMENT AND CONFIRMATION OF REGISTRATION
All correspondence will be by email, please ensure that the email address is written clearly on the registration form. Acknowledgement of receipt of registration will be emailed to delegates.
Confirmation of registration will only be sent on receipt of appropriate payment by the Executive Secretariat. Where no email address is given, both the acknowledgement and confirmation will be by ordinary mail. The Executive Secretariat cannot guarantee delivery of ordinary mail.
Delegates who have registered, paid and not received confirmation within 14 days of payment, should contact the EACTS Executive Secretariat.
Delegates should check the details on the confirmation carefully and contact the EACTS Secretariat immediately if there are any errors. Spelling or literal errors will be amended upon request until 18 August 2006. After this date and on-site there will be a charge of € 25 for amending and re-printing name badges.
CHANGES TO INDIVIDUAL REGISTRATIONS
Please inform the Secretariat immediately if there is an error in your acknowledgement or confirmation of booking.
Delegates may make additions to their registration at any time before 18 August 2006 by emailing or faxing the EACTS Secretariat with their request. The extra activity will not be added to the registration until full payment is received. Delegates should quote their registration number in all correspondence.
Amendments to existing registrations will be subject to the following fees:
Transfer of Registration:
on and before 18 August 2006 - € 75
after 18 August 2006 - not possible
Session Change:
e.g. changing from Adult Cardiac to Thoracic sessions within Postgraduate course or Breakfast Session - no fee
Activity Change:
fee as per cancellation fees for activity cancelled.
CANCELLATION
Cancellations must be made in writing to the EACTS Executive Secretariat and received before 4 August 2006. The fee(s), less a 25% administration charge, will be refunded. It is regretted that after this date no reimbursement of fees can be made. (The 'Early Member fee' administration charge of €10 is non-refundable)
Cancellation of a double booking must be made in writing to the EACTS Executive Secretariat and received before 18 August 2006. The fee for second booking will be refunded, less a 25% administration charge. (The 'Early Member fee' administration charge of €10 is non-refundable).

Pre-registered delegates should collect their delegate packs from the Registration Area, Stockholm International Fairs on:

- Transfer of Registration is not allowed on-site
Transfers will be treated as new registrations and will be charged at the on-site registration fee. - Spelling or literal changes to badges will be charged at € 25 on site. Please check these details when you receive your confirmation of booking.
- A fee of € 100 will be charged for replacement of lost badges.
